Why did I get two tax filing documents (1099-R or 1042-S forms)?

There are several reasons why someone could receive two 1099-R forms. Below are the most common scenarios. 

  • If you have turned age 59 ½ during the tax year in question, you are considered normal age for retirement and your distribution code on your 1099-R form will become a "7." We are required to send two 1099-R forms to you and the Internal Revenue Service (IRS) to separately report the portion paid to you before age 59 ½ and the portion paid to you after age 59 ½.
  • If you have moved to a new state during the tax year in question, we are required to send two 1099-R forms to you and the IRS to separately report taxes according to your state residency.

When will the tax filing documents be sent?

Your tax filing document (1099-R or 1042-S forms) will be mailed by January 31, following the tax year. For an example, your 1099-R form for 2013 will be sent out in January of 2014. Your tax filing document will be sent to you via email if you have registered online. If you elected not to have your tax filing document emailed, then your tax filing document will be sent to your home address on file.

How do I request a reissued tax filing document for a prior year?

As of the tax year 2014, you can view and print your tax filing document (1099-R or 1042-S forms) for previous years online by logging in to your account opens in a new window, using your user identification and password. You also can request your 1099-R forms for previous years by contacting Prudential Financial at 1-800-336-3387.

How can I tell where my tax filing document was sent?

What is this statement I received in June and December?

A semiannual statement is sent to all of our retirees with the June and December payments. This statement shows the total amount you have received for the year and the amount you have withheld for taxes. This statement is for your record keeping purposes only. It is not a bill nor is it a tax filing document.

Do I get a monthly statement?

A semiannual statement is sent to all of our retirees with the June and December payments. If you need a verification of income, you can call Prudential Financial at 1-800-336-3387 to request a Pension Verification Letter. A Pension Verification Letter will outline the date you started receiving your benefit, the duration of your benefit, and the amount of your benefit.

Can I get a copy of my statement? I didn't get it.

If you are applying for a loan or medical insurance, you can call Prudential Financial at 1-800-336-3387 to request a Pension Verification Letter. A Pension Verification Letter will outline the date you started receiving your benefit, the duration of your benefit, and the amount of your benefit.
